Key Responsibilities
- Deliver clinical nursing care to aged care clients and NDIS participants at home
- Lead clinical assessments and the development and review of care plans
- Oversee medication support and monitor clients for changes in health
- Keep accurate clinical documentation
- Work in partnership with clients, families, carers and healthcare professionals, championing choice, dignity and independence
Requirements & Eligibility
- Current AHPRA registration as a Registered Nurse
- Relevant experience across community nursing, aged care or disability services
- Strong clinical assessment, communication and organisational skills
- Confidence working independently in a community setting
- A compassionate approach, a commitment to safe, person-centred care and the willingness to move to the Northern Territory
